Description: Situation Log is used to record resident's poor behaviour.


Please follow the guided instructions below on how to add a record in Situation Log


1. Go to Rewards > Situation Log.


2. Under Record, click the Add button to add a record into the table.


3. Fill up the basic information.


4. Describe how the situation began.


5. Describe the situation.


6. Select how the situation end.


7. Describe follow-up action.


8. If the situation reaches R1, click Show-R1

If the situation do not reach R1, select the staff who is completed the situation and the staff to endorse, and click Save.


9. Select 'Yes' if the resident enter this stage. NOTE: if resident did not enter this stage, click Hide R1 to close R1 form.

Select the duration of this stage, reaction of the resident in this stage.

Optional: Describe the stage.


10. If the situation reaches R2, click Show-R2/R3

If the situation do not reach R2, select the staff who is completed the situation and the staff to endorse, and click Save.


11. Select Yes' if the resident enter R2.  NOTE: If resident did not enter this stage, click Hide R2/R3 to close R2/R3 form. 

Select the duration of this stage, reaction of the resident in this stage.

Optional: Describe the stage.


12. Select 'Yes' if the resident enter R3. NOTE: If resident did not enter this stage, leave 'Did resident enter this stage' as 'No' and skip R3 form.

If resident enter R3, select the duration and reaction of the resident in this stage.

Optional: Describe the stage.


13. Select the staff who is completed the situation and the staff to endorse, and click Save.
